Request Full Records
Contact your GP to ensure Assess2Care can access your full medical record. Our advice is to send the email template to your GP and then phone the reception and request full access to your medical record.
Please note: The assessment will not be able to progress until this is provided. If your practice requires a written request for full records, please use the resources below.
